It keeps telling me incomplete information and I have filled out all the available boxes. And now the rate this app won't submit. Poor user experience.

First time logging in, and I'm immediately required to reset my password, for the account I just set up on the site. After selecting my new password and typing it again, I hit Submit and get annex to my uneditable email address, with no way to get past this point.

Didn't recognize city and zip code on my address. Other consent forms for new patient says must complete at the office. What's the purpose of the forms on the app then?
