Horrific. -5 negative stars. The interactive map is just broken. If you select "Vision", the dental providers are flagged on the map. If you select "Dental", you get hearing providers- and, you guessed it - if you select "Hearing" you'll get vision providers. There is a map scale slider from 1 to 50 miles. It doesn't actually do anything. If you select "View all providers", you get to fill in a form that makes you scroll through 500 cities then asks for county. DUMB. Absolute waste of coding.

I was made aware of this app from a CarePlus csr as my mom has requested her care card 4 times and still has not received it, the csr failed to mention you need the care card number to use the app!
